4.BIRTH CERTIFICATE

⑴If there is “no record” in PSA, ・Certificate of Non-Record” from PSA with “Birth certificate from Local Civil Registrar”. ⑵If there is “Late registration” in PSA ・Birth certificate from PSA with “BAPTISMAL CERTIFICATE” , “SCHOOL RECORD(FORM137) from elementary or high school” and “SCHOOL YEARBOOK(if possible)” ⑶If there is unreadable in PSA ・“Certificate of Non-Record” from PSA with a “Birth certificate from Local Civil Registrar”. |

5.MARRIAGE CERTIFICATE

⑴If the applicant is married. ⑵If there is “no record” in PSA ・“Certificate of Non-Record” from PSA with “MARRIAGE CERTIFICATE” from Local Civil Registrar. |
