1.PASSPORT
⑴Validity more than 6months from return date  
⑵NOT ACCEPT under below   
 ①Broken lamination of the Picture Portion   
 ②No signature   
 ③Less than 2blank pages left in the passport
2.VISA APPLICATION FORM
⑴Fill in English
⑵If not Fulfill “N/A” (Please no Omitted)
⑶Please don’t be use correction.
⑷Not Accept specially NO DATE, NO SIGNATURE.
⑸May not issued visa when there is a writing error.
⑹Not issue visa if falsehood information.
⑺Date is “APPLICATION DATE” or “CREATED DATE”
3.PHOTO

(4.5cm ×4.5cm, white background)

⑴No caps, must be within 6MONTH after take photo.  
⑵Uncensored & clear  
⑶Not Accept case by not clear, aggrandize photo  
⑷Please write on back side ”NAME” & ”BIRTHDATE”
4.BIRTH CERTIFICATE

(Issued within 1year from PSA/ ORIGINAL)

⑴If there is “no record” in PSA,
 ・Certificate of Non-Record” from PSA with “Birth certificate from Local Civil Registrar”.
⑵If there is “Late registration” in PSA  
 ・Birth certificate from PSA with “BAPTISMAL CERTIFICATE” , “SCHOOL RECORD(FORM137) from elementary or high school” and “SCHOOL YEARBOOK(if possible)”  
⑶If there is unreadable in PSA  
 ・“Certificate of Non-Record” from PSA with a “Birth certificate from Local Civil Registrar”.
5.MARRIAGE CERTIFICATE

(Issued within 1year from PSA/ ORIGINAL)

⑴If the applicant is married.
⑵If there is “no record” in PSA  
 ・ “Certificate of Non-Record” from PSA with “MARRIAGE CERTIFICATE” from Local Civil Registrar.
※can’t without 4 and 5 if applicant already have used Japan visa
6.EXPLANATION LETTER of REASON TO ENTRY JAPAN
⑴from Japanese spouse /Parent living in the Philippines
7.FAMILY REGISTRATION

(Koseki-tohon /issued with in 3months from City hall / Original)

⑴if requesting a multiple entry visa
8.PASSPORT COPY of Japanese Spouse / Parent Living in the Philippines
⑴Please submit a copy of all used pages to confirm that the Japanese Spouse/Parent is living in the Philippines with long term visa. If the Japanese Spouse/Parent is staying in the Philippines on a short-term visa such as “Tourist”, he/ she is not recognized as a longterm resident therefore Spouse/Child cannot apply in this category.
9.Copy of ALIEN CERTIFICATE OF REGISTRATION(ACR) card of the Japanese Spouse / Parent
⑴clear photocopy back to back
10.EMPLOYMENT CERTIFICATE and INCOME TAX RETURN
⑴If unemployed or only receiving pension etc., submit a written explanation why the main provider cannot submit this requirement.
⑵If main provider of the family can’t submit Income Tax Return (ITR), submit a “BANK CERTIFICATE” in lieu of the ITR.
